Medical Case Management

Modern Assistance Programs, Inc. (MAP) established a medical case management division in 1994.  This service is only available to certain unions. Please check with your benefits book or fund office to find out which services your union is covered for by MAP, Inc.

The licensed health care professionals at MAP, Inc. offer a Patient Assistance Program that include the following components:

-  Medical Pre-Certification for hospital admissions, day surgeries, hospice or home care services, and outpatient rehabilitative services. When a Pre-certification process is required to access insurance coverage, the patient’s needs are brought to the attention of the MAP multidisciplinary case management team at the start of the patient care process.

-  Medical Case Management addresses utilization review, coordination of care, and discharge planning. Modern Assistance Programs, Inc. uses a holistic, patient centered philosophy, which engages the patient, family, medical providers and community and public resources in promoting quality and cost effective out-comes. This process empowers the individual, which leads to informed medical decision making, improved quality of care, better access to services and health benefit conservation.

-  Medical Social Work services address the bio-psychosocial needs of patients in the context of family, community, employment, legal, medical, and language/culture.  Medical social work provides much needed assistance that patients and their families need during times of crisis. A MAP clinician does the following:

• Works with patients and family members who face profound changes in their lives due to illness and/or death.
• Identifies and links patients and families with community and government resources
• Provides short term counseling at MAP/home as needed
• Provides educational support about current treatment trends, wellness programs, psychological supports                                     

-  Medical Disability Case Management

Assists patient’s with the application process for short term and long term disability benefits as needed and ease the transition back to work by interfacing with employers as needed.

-Alternative and Complementary Treatment Management

Some unions have expanded their benefit packages to include coverage for alternative and complimentary therapies, including, acupuncture, chiropractic care, massage therapy, homeopathy, naturopathy, nutritional counseling, and hypnotherapy. Please refer to your insurance manual to determine if this benefit is available to you.
